Oral conditions such as velopharyngeal closure relate to playing instruments and also to singing, and mandibular conditions can be related to playing string instruments such as a violin.



According to recent reviews, musicians can experience occupational orofacial problems caused by excessive practice or stress and dentists may address these issues.

As for the Class III malocclusion, clinicians need to understand the age-appropriate treatment methods. 

In growing children around six years old, which means IIIA or IIIB in dental age, maxillary protraction appliance (MPA) is used in the patients with maxillary retardation. 

MPA contributes to the maxillary growth forward and downward by stimulating the sutures in the maxillofacial region combined with some additional appliances, for example, rapid maxillary expansion for the maxilla and Ti-made anchor plates for the mandible.



In adolescent patients, functional appliances are used, and the mandibular growth direction will be changed clockwise by extruding the molars. 

This helps Class III patients to increase the ANB angle by decreasing the SNB angle.



In adults, we apply modified MEAW technique using Improved Superelastic Wires (ISW) that we invented to upright the lower molars effectively without any complicated loops. 

Another characteristic of the wire is low hysteresis compared to the other Ni-Ti wires.

In this century, temporary anchorage devices (TADs) are often used to move the lower teeth distally without causing any reaction in the maxillary teeth.



If significant skeletal discrepancy remains in adults, we should choose orthognathic surgery such as sagittal splitting ramus osteotomy with or without Le Fort I osteotomy. 



The number of these patients has been increasing year by year, especially in these two decades.

 It can improve the skeletal relationship sagittally and frontally. Recently, we have combined digital dentistry with our conventional methods. 



A CAD/CAM splint is used as the intermediate splint for repositioning the maxilla more accurately to the planned position utilizing CT images and 3D simulation.

In this talk, I will present the age-appropriate treatment method for Class III malocclusion, dividing it into the following 4 sections:

About:

Improving the aesthetics of anterior teeth can have a positive impact on patients, both socially and psychologically. 

In daily practice, prosthetic approaches are one method, however, there are other ways to achieve aesthetic improvement.

For example, in many cases, soft tissue management is used to improve the aesthetics of gingiva. 

In this presentation, I would like to cover the following points:

 

・Anatomical features of natural dentition and periodontium  

・Evaluation criteria of anterior aesthetics  

・Hard and soft tissue management for achieving aesthetic balance



First of all, having basic knowledge of anatomical and histological features of natural teeth and periodontium helps us to understand anterior morphology. 

The aesthetics level can be evaluated by plotting each anatomical position and checking its symmetry and continuity to adjacent teeth.

In daily practice, prosthetic approaches are one method, however, there are other ways to achieve aesthetic improvement.

For example, in many cases, soft tissue management is used to improve the aesthetics of gingiva.

But as you all know it's very difficult to be a good prosthodontist if you don't have a good technician by your side.&nbsp; And sometimes I'm joking that many Prosthodontists have a chance to change their dental technician but I cannot change mine because mine is my father and I'm really honored to cooperate with him and because you know it doesn't matter how good you are as a Prosthodontist but you will not be able to achieve anything if you don't have a good dental technician and a good team overall. And at the beginning you know my father was doing metal crowns then he was doing metal ceramic crowns, then there was era of pressing ceramics, and then in 2016 we together decided to move to the digital workflow.&nbsp; And again today we will also share with you the details how we manage the digital workflow in the lab and we will show you the videos of all of the procedures from that workflow.

Implant overdentures (IOD) have a long history of use, and various clinical studies have been conducted since the 1990's on outcomes. The McGill Consensus in 2002 and the York Consensus in 2009 established the 2-Implant Overdenture (2-IOD) as the standard treatment for an edentulous mandible. The need for this minimally invasive and low-cost dental treatment has been increasing along with the number of super-aged societies in the world. In edentulous patients, there are several types of attachments for implant overdentures, e.g., ball, Locator, magnetic and bar. Each has its own advantages and disadvantages. The advantages of Locator attachment, for example, are selective retentive force, lower height to abutment, and less rotation of dentures. However, placement of dentures can be difficult for older patients who have poor dexterity or are under nursing care.
